Guest Agreement

Introduction

ABODE offers safe, welcoming, short-term, personal care for Guests entering the last 3 months, weeks, or even days of their lives. We are not a medical facility or a nursing home. We are a family-like home where our well-trained staff, volunteers, and doulas give shelter, meals, and personal, spiritual, and contemplative care 24 hours a day, 7 days a week.

ABODE does not offer skilled nursing care or medical services. Your licensed hospice agency and designated physician provide those services.

Think of us as your extended family – we help carry out your hospice’s care plan, and we appreciate the assistance of your loved ones in caring for you, especially on weekends and holidays.

We’re often asked how we provide our extraordinary level of support at no cost to hospice or our Guests. While it’s true that we do not charge for our care, our work does, indeed, require money. We estimate that our care costs approximately $300 per day, or around $ 9,000 per month, for each of our Guests.

Therefore, we respectfully ask that our Guests and their families and friends consider helping with costs to their level of ability. An upfront gift is extremely helpful to us, and gifts made every two weeks during a Guest’s stay are also greatly appreciated.

We also very much appreciate obituary mentions. And we accept checks and online donations: abodehome.org/donate.

Admission to ABODE, however, is not based on income level or ability to pay. All are welcome at ABODE.

All Guests, their responsible party (RP), Medical Power of Attorney (MPOA), or legal representative must read, understand, and sign the following Guest Agreement and return it to ABODE.

Welcome to ABODE! We are honored to care for you!

Address of the person filling out this form.
  1. I am under the care of a licensed hospice agency. My hospice nurse will be with me upon admission to ABODE. I have a Responsible Party (RP), Medical Power of Attorney (MPOA), or a lawyer who will sign this document as well, either in person or remotely.

  2. I have been diagnosed with a terminal disease/condition with 3 months or less life expectancy. No life-extending treatments will be offered at ABODE – ABODE will collaborate with my hospice to provide comfort care only.

  3. I understand that ABODE does not necessarily replace, but rather extends, the ability of my loved ones to provide care. My family and friends will assist ABODE in my care – if possible, and under ABODE’s direction – especially on weekends and during holidays. This can include regular visits with me, bringing my favorite foods, and contributing financially to my care.

  4. I understand that if I have a concern about my care, I will bring it to the attention of the ABODE staff. ABODE can be of some assistance, but key decisions must be made by my hospice and me.

  5. My medications will be dispensed by ABODE staff as scheduled or as needed, under the direction of my physician and hospice nurse.

  6. I give consent and approval for the release of information to or from ABODE and any physician, nursing facility, hospital, my hospice, my family members, or others involved in my care. My hospice team and the ABODE staff have my consent and approval to communicate freely at any time.

  7. Smoking is not encouraged, but is allowed only on the porch with my patio door and window closed. If other parties are outside, refrain from smoking. No smoking is allowed inside ABODE. Alcohol is not encouraged, but exceptions can be made on a case-by-case basis.

  8. House-trained pets are welcome to visit on a limited basis. Dogs and cats must be on a leash; all other pets require permission from management.

  9. At the 2 ½ month mark (or sooner, at ABODE’s request), my hospice team will reevaluate my life expectancy.

  10. If my condition stabilizes or improves, if ABODE needs to briefly close or in the unlikely event that I contract COVID-19, my hospice and I will arrange for me to be relocated.

  11. The peaceful, contemplative nature of ABODE means that everyone treats one another kindly. Neither I nor my family/friends will mistreat any ABODE staff, volunteers or doulas. Threatening, aggressive or disrespectful behavior is not appropriate at ABODE. All communications must be shared in a respectful tone. Intentional failure to communicate properly likely means that ABODE is not the right place for me, and I will work with my hospice to move to a different facility.

  12. I will collaborate with my family, friends and/or hospice to provide ABODE with my funeral arrangements. The ABODE staff, volunteers and doulas are happy to assist, as needed.

  13. Visiting hours are from 08:00 am - 10:00 pm. Only two visitors are allowed to stay overnight in the Guest’s room and must be in-house by 09:30 pm.

  14. I have reviewed and commit to abiding by the ABODE Guest Agreement and have been given the opportunity to ask any questions.

  15. Media Opt Out: As part of ABODE’s effort to raise awareness and funding for our good work, we often write about, photograph and video our Guests, family members and friends, and share their stories and ABODE testimonials – with their consent. We use these pieces for our website, social media, brochures, newsletters and any number of other promotional pieces. And from time to time, we share these photos/stories with the media for wider distribution.
